Mr. Ji- Motor Neuron Disease (ALS, Male, 54 years old)-2005/2006

Mr. Ji is a Chinese 54 years old male patient suffering from motor neuron disease (ALS). His disease onset was from May, 2004, then he was diagnosed as motor neuron disease in local hospital in March, 2005. He received 1st round stem cell treatment in our hospital in November, 2005, 1 year after the disease onset; he received 2nd round stem cell treatment in our hospital in March, 2006, 2 years after the disease onset.

His brief medical history and general condition:
He underwent fasciculation of both upper limbs without any incentive in May, 2004, but he could move his upper limbs normally, so he did not receive any special examination and treatment. The fasciculation of his both upper limbs was more frequently occurred with fasciculation of his lower limbs in October, 2004, so he consulted to local hospital, EMG indicated: no abnormal, he was not given proper diagnosed and treatment as final. The fasciculation of his 4 limbs was aggravated obviously with movement dysfunction of his lower limbs in December, but he was not given the symptomatic treatment too. He underwent the weakness of upper limbs, could not lift up his upper limbs with his escaping muscle atrophy in February, 2005, so he went to the local hospital for consultant, the EMG indicated: no pathological spontaneous electric potential, then was diagnosed as motor neuron disease whilst was given Riluzole for the treatment, but he did not accept it. He could not lift up his lower limbs, walk difficultly, for further definite diagnose, he went to Beijing hospital for the consultant. Based on the results of EMG and MRI, he was diagnosed as motor neuron disease and cervical spondylosis but did not received any special treatment. Then he received EMG in Beijing 3rd hospital and was diagnosed as motor neuron disease. He took Riluzole by oral for over 1 month without obvious improvements. He took Chinese medicines since May, 2005, the efficacy was not good. His disease was progressive, which affected his upper limbs, truck muscles while no choke when drinking with clear speech.

Before 1st round stem cell treatment:
- He could turn his head weakly, could not shrug, obvious trailer syndrome in his upper limbs, escaping muscle atrophied, big and small thenar muscles atrophied and shoulder muscle atrophied;
- The muscle strength of his distal left upper limb was 2 grades, the proximal muscle strength was 0 grade; the muscle strength of his distal right upper limb was 2 grades, the proximal muscle strength was 0 grade;
- The muscular tension was normal, the tendon reflex was weak, bilateral Hoffman sign (+); the muscle strength of his lower limbs was 3- grades, the muscular tension was normal, the tendon reflex was normal;
- The right Babinski’s sign (+), soft neck, Kernig’s sign (-), the deep and superficial sensation, pain feeling and temperature sensation of whole body were normal, the propriocepiton was existed, the temperature of his hands and feet was low.
- Regarding to assistant examination, EMG in Beijing University 3rd hospital of January, 2003 indicated: extensive neurological damages.

During the 1st round stem cell treatment (after 4 stem cell implantations):
- He could swallow better than before;
- The fingers of his left hand could stretch straight;
- He could lift up his right arm;
- He could walk stronger than before.

His general medical condition before 2nd round stem cell treatment:
- His speech was slurred;
- He choked occasionally during drinking or eating;
- He breathed difficultly when lying on back;
- His neck was too weak to lift up or straight up his head;
- His upper limbs was too weak to lift up;
- His both hands could not make a fist;
- The muscle strength of his lower limbs was weak, so that he could walk a short distance by other’s assistant.

The general physical examination before 2nd round stem cell treatment:
- His speech was slurred;
- His head turned weakly, he could not shrug, he suffered from obvious trailer syndrome of his upper limbs;
- His escaping muscle atrophied, big and small thenar muscles atrophied and shoulder muscle atrophied;
- The distal muscle strength of his left upper limb was 2 grades, the proximal muscle strength was 0 grade; the distal muscle strength of his right upper limb was 2 grades, the proximal muscle strength was 0 grade;
- His muscular tension was normal, the tendon reflex was weak, bilateral Hoffman sign (+); the muscle strength of his lower limbs was 4 grades, the muscular tension was normal, the tendon reflex was normal, the right Babinski’s sign (+), soft neck, Kernig’s sign (-), the deep and superficial sensation, pain feeling and temperature sensation of whole body were normal, the propriocepiton was existed.

His general medical condition during the 2nd round stem cell treatment (after 4 stem cell implantations):
- He can breathe more easily when lying on back than before;
- He can straight his head for a period;
- He says that he can feel his right lower limb more stronger and relaxed;
- His speech is much clearer.
